Franklin County Public Health offers adult and child immunizations by appointment only. To schedule an appointment, call (614) 525-3719 or visit www.myfcph.org. Most major insurances are accepted. A sliding fee scale is available for those with no insurance.
You must to bring the following to the appointment (if applicable): shot record, parent photo ID and insurance card. Children cannot be seen without these three documents. A completed consent form is also required if a parent or legal guardian is not present at the clinic. Most appointments are scheduled between 9:40 a.m. to 2:00 p.m.
