Franklin County Public Health adheres to all Ohio Public Records Laws.
In accordance with the Ohio Revised Code section 149.43, Franklin County Public Health is required to make records available for public inspection within a reasonable time period. Additionally, in accordance with HIPAA, no personally medically identifiable information will be released via records requests.

If you would like to make a request for public records, access with the Request Portal button below. If you would like to request an immunization record, complete the Immunization Record Release Form instead.
