Complex Medical Help Program (CMH)

What We Do

The CMH program helps families of children with special health care needs. CMH works like a back-up insurance for medical services for eligible children under the age of 25 years old in Ohio.

How We Help

Families on the CMH program are given a Public Health Nurse (PHN):

  • A PHN is a registered nurse who works with children and families in the community.
  • PHNs talk with families through phone calls, text messages, emails and home visits.

PHNs provide free services to your family, including:

  • Document iconExplaining the CMH program application and other paperwork.
  • Helping you learn your child’s medical treatment plan.
  • Finding you CMH providers for needed medical visits.

PHNs also help with:

  • Connection nodes iconFinding and referring you to community resources.
  • Child iconHelping you learn about your child’s growth and development.
