The Care Coordination program connects you to a Community Health Worker (CHW) who offers many services to help you with your medical and everyday needs.

How We Help

How We Help:

  • A CHW is a certified public health worker who lives and works here in Franklin County and knows our communities.
  • We assign you a CHW.
  • CHWs serve as a link between you and service providers in the county.

The CHW will enroll you in the program, where they:

  • Assess your goals, needs and limits to getting medical and social services.
  • Meet you monthly in-person and reach out to you through phone calls, text messages and emails.

CHWs offer free services to you, including:

  • Helping you finish health insurance applications and describe insurance to you.
  • Helping you find food assistance, utility and financial assistance, and career and job resources.
  • Finding you a doctor or other medical services (dentist, eye doctor, behavioral health).
